Our objective at In Form Chiropractic is to determine if you have
vertebral subluxations (nerve pressure) interfering with your body's ability
to function at 100%.
On your first visit to our office you will first be asked to fill in
a health questionnaire that will provide us with much needed and important
information. You will then be shown a video to give you some information
about chiropractic, that we feel is important for you to know, before
seeing one of our chiropractors. Once you have seen the video one of our
chiropractors will guide you to the consultation room so they may obtain
information about your health history and determine how chiropractic may
be beneficial to you. They will also provide an explanation of chiropractic
and how it relates to you attaining optimal body function.
Once this has been done, a thorough chiropractic evaluation will be performed.
This evaluation will include physical evaluation, computer heat scan and
spinal EMG ( electromyography ). This evaluation will allow the chiropractor
to determine if you have vertebral subluxations (nerve pressure) present
affecting your body's ability to function correctly.
At this point a decision will be made, by the chiropractor, as to whether
x-rays of your spine are required prior to you undertaking care. If x-rays
are taken the chiropractor will require time to review them and correlate
all information obtained to this point. We will then schedule you for
a Report of Findings.
At your report of findings the chiropractor will explain where he/she
has found vertebral subluxations (nerve pressure areas) in your spine
and how serious your situation is. A plan of care will then be outlined
for you based on your individual needs and the stresses that you encounter
in your everyday life.
